Feedback forms

• Create a practice feedback form – compliments, complaints and suggestions (or review existing forms) that encourages feedback from the LGBTIQ+ community and all.

• Does the feedback process in place at your practice allow for equitable feedback from all patients?

• Encourage input from patients who identify as LGBTIQ+ to ensure form is appropriately inclusive.

  NB It is recommended that the modules on Knowledge and Understanding are completed prior to undertaking this task, or that your practice are already confident in their knowledge and understanding.

Evaluation form

• Create an evaluation form to assess how you are performing from an inclusive practice perspective. An evaluation form is different to a feedback form as you can ask particular questions to assess your practice. Consider how to distribute the evaluation form / survey. You may ask any patients who identify as LGBTIQ+ to complete the evaluation on a voluntary basis or you may display the survey in your practice waiting room.
